Our collaboration with NACCHO, ASTHO and CDC

We’re proud to work with the Getting Further Faster initiative because they:

  • Address root causes to prevent chronic disease by focusing on social determinants of health (conditions where people live, work, and play)

  • Work on the ground in communities to increase access to resources from multiple sectors (public health, health care, community organizations) and advance health equity

  • Are building an evidence base of community coalition and multi-sector partnership success stories for peer learning 

 
 

The Getting Further Faster initiative is a partnership between CDC’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), the National Association of County and City Health Officials (NACCHO), the Association of State and Territorial Health Officials (ASTHO), and community-led partnerships across the country.

 

Opportunity

  • Inspire and educate community coalitions and partnerships across the U.S. about how to effectively address social determinants of health (SDOH) through the sharing of success stories.

  • Better position coalitions and partnerships to effectively solicit support and funding to sustain their work.    

Approach

  • Discovery process to identify coalitions and partnerships that are effectively addressing SDOH

  • Storytelling and narrative-building to bring success stories to life and showcase the voices of people doing the work, which engages audiences and inspires action

  • Video series and print materials for compelling multi-purpose content 

Health+ Studio identified and collaborated with 14 community coalitions and multi-sector partnerships throughout the U.S. to produce story and impact videos that could be used for multiple purposes, including education and outreach, fundraising, and peer learning. Health+ hosted a webinar and led coaching sessions to help coalitions clarify their “why:” What is our mission and vision, what is the story we want to tell, and how are we making an impact? Health+ also helped coalitions develop messaging and identify which partners should be the storytellers to most effectively showcase the work. 

Health+ helped identify common threads of success woven throughout the coalitions’ work that others can learn from, regardless of sector or geography. The videos are categorized by the five domains of SDOH (a healthy built environment; connections between communities and clinics; access to nutritious, affordable food; social connectedness; and tobacco-free spaces). 

The video series is featured in the video hub on Getting Further Faster’s  Community of Practice, a peer-learning community that provides resources, tools, trainings, and support to coalitions/partnerships working together to create healthy communities. Print materials were developed to help each coalition and partnership reach its specific goals, from community education to fundraising. 

 
 

Outcomes

  • Compelling impact videos and print materials that showcase each coalition’s story, expertise, and impact

  • Collection of core materials for the Getting Further Faster’s Community of Practice, which makes learning and insights accessible to other communities across the country

  • More effective coalition fundraising through stories of transformation and qualitative and quantitative impact metrics
